Visa application form fully completed and signed by the applicant.
Travel document (passport) that must have a validity of at least three months and two blank pages.
Detailed covering letter explaining the purpose of visiting the Czech Republic.
Two recent photographs (35 x 45 mm) with white background and 80% face close up.
Proof of funds for your trip (bank account statement for the last 3 months, copy of fixed deposit if any).
ITRs of last 3 years or Form 16.
An employee needs to submit: Job appointment letter, confirmation letter, NOC from an employer stating your employment information and leave.
An employer needs to submit the business registration papers.
Return Air Ticket that should be confirmed.
Accommodation booking for the period of your stay in the Czech Republic.
Itinerary for the number of days of your stay.
Travel Health Insurance for at least Euro 30,000.
