Essential (common) Documents for All Applicants

• Travel document (Passport) of the applicant having a validity of minimum 3 months extending the return from Schengen country.
• Round ticket of flight in order to confirm that the applicant is not having the intention of staying in Poland for more than a specified period. If he/she is intending to visit multiple Schengen countries, there's a need to enclose the proof of transportation (flight or train) confirmation.
• Two latest photographs with 35×45 dimension, 80% face closure. Editing of photos with any kind of software is prohibited.
• Confirmed accommodation (hotel, hostel or holiday home). If the applicant is planning to stay with a family member or a friend, he/she will require to get an invitation from the host member and submit it to the embassy.
• Bank account statement for the last three months, fixed deposits (if any), proof of income or savings in the form of investment policies or more. If the expenses would be met by the company, statement by the host company required on the official letterhead.
• Income Tax Returns of the last 3 years along with the supporting documents.
• Travel history details.
• Travel itinerary stating the schedule of your entire trip to Poland.
• Travel health insurance of Euro 30,000.
• An employed applicant needs to submit NOC from the company, payslips for the last 3 months, and employment contract.
• Self-employed or business person requires to submit the registration proof of the company, GSTRs, and current account statement.

• Copy of the travel documents (passports) of both parents or guardians.
• Written consent of both parents or guardians if the applicant is traveling alone or with someone else.
• In case the minor is travelling with one parent, there's a need of written consent of the other parent.

• Detailed itinerary of the trip.
• Confirmation of the organized trip by the registered travel agency.

• Invitation from the polish company stating the date of visit, identification details of an applicant, and address of the company.
• Covering letter from the company of Poland.

• Attendance record from the school or college.
• NOC letter stating the leave.
